In Arizona, physicians can determine if you qualify without prior medical records, but having them handy can help you get more out of your doctor’s evaluation.

When it comes time to apply for your med card, you’ll need to show proof of Arizona residency and identity. An Arizona state ID or your driver’s license works fine. You’ll pair that with your physician certification/recommendation to apply for a card. You’ll also need any SNAP documentation or other proof of need for financial assistance, if relevant.
