Great question! Here's how it works:
First, you do need to have a FL state ID, or be a seasonal resident with proof of FL residency in order to apply.
You'll complete the online pre-screening application and answer a few questions about your health (all of the forms only take a few minutes). If your address is very far from one of offices we'll let you know. Then you'll upload your photo ID and submit payment information.
While medical records are not always required, your doctor may ask to see some so please be prepared to share those if requested. If you do want to share anything, an option to do so is provided in the application.
Once that's complete the doctor will review your application and determine if you can be pre-approved. The hold on your card is immediately voided if you are not approved by the doctor, so there's no risk!
Once you are pre-approved online, you'll receive a notification to schedule your in-person evaluation at the doctor's office. We have convenient locations in Tampa, Miami, and Fort Myers. After you meet with the doctor and are approved, they will certify you online through the state's system. You may then complete your registration online and receive your card.
