An evaluation with our doctor is required every year. Your initial evaluation costs $159. After your first visit, renewals are $125 per year and can be completed via telemedicine with the same provider.
Kentucky charges a $25 annual fee for both patient and caregiver medical marijuana card applications. This fee is separate from the cost of your medical evaluation and follow-up appointments.
