Caregivers must register with the state in the same way as patients, caregivers will do this on their own via the state.
After completing the caregiver application, submitting all required documentation including copy of valid ID (DL, State ID, or Passport) and the patients written certification.
Pay a fee of $25. Caregivers must then download and print the signature form, notarize and upload the notarized document. The Cabinet will notify you of approval or denial within 30 calendar days. Approved applicants will receive their registry ID card electronically within 5 calendar days.
