To become an MMJ caregiver in West Virginia, your patient must fill out this Patient Authorization for Designation of Caregiver. The form must be printed and filled out in full with a notarized signature. Once your patient has filled out and notarized this form, they will give it to you, the designated caregiver, to upload with your caregiver application on West Virginia's medical marijuana registration portal. This application will require a criminal background check as well.
