The cost for the evaluation with our doctor in IL is $99. There are three options when you register with the state, you can choose a 1 year license for $50, 2 years for $100, or 3 years for $125. Patients enrolled in SSI/SSDI, US Railroad Retirement Board Disability, Illinois Teachers’ Retirement System Disability, Illinois State Universities Retirement System Disability, or a Veteran qualify for a reduced fee of $25, $50, or $75 for a 1, 2, or 3 year license respectively.
