First and foremost, registration with the state is not required to claim the protections of the state's medical marijuana laws. You only need a valid, written certification from your doctor. If you wish to voluntarily register with the state, a completed medical use application is necessary. For this application, you'll need to provide:
Name, address, and date of birth of the patient.
A copy of the written certification issued by the qualifying patient's physician for the medical use of marijuana.
A copy of your Maine driver's license or other state-issued photo identification.
Patients who elect to cultivate some or all of the maximum allowed amount of marijuana for their own medical use must indicate that choice on the application.
